Frequent yellowish discharge, burning sensation and whitish discoloration of the skin or the prepuce indirectly indicates it is balanitis or preputial skin infection. Yellowish discharge means ther is infection inside the urethra leading to pus discharge which is yellowish in colour. Whenever there is a discharge, trauma to the mucosal layer, indirectly causing a wound and urine over the wound causes burning sensation. When you say there is a urethral discharge, pain in the tip of the penis that means the meatus has also got an ulcer or a wound and then the skin over it causes a balanitis. It is called as balanoposthitis when it is completely heals by contracture. That means when the wound that is there, it heals by contracture, leading to more of phymosis and whitish discharge which is causing damage to the skin. So these patients has to be treated for the infection first and if the skin is tight then circumcision has to be done and a lot of hygiene has to be maintained, this usually occurs in older age group of people. So control of infection, hygiene and plenty of fluids will avoid you from these circumstances and when, balanitis, is there, we have to think if the patient is diabetic. An immunocompromsed patient should also be thought about.

A middle ear infection is a condition that occurs when a virus or bacteria causes the air-filled space behind the eardrum to become inflamed.
• Also called otitis media, a middle ear infection is the most common type of ear infection individuals face.
• This condition is more prevalent in children than in adults. The Lucile Packard Children’s Hospital at Stanford estimates that middle ear infections occur in 80% of children by the time they reach age 3.
• It is easier for parents to recognize the condition in children who are able to articulate how they are feeling than in infants and toddlers.
• Hearing problems and other serious complications can develop from long-term exposure to ear infections – persistent or frequent ear infections, persistent fluid in the middle ear.
• These complications may include impaired hearing, the spread of infection, tearing of eardrum, and even speech and developmental delay.

Some other conditions that may result in similar middle ear problems or that may be related to an ear infection may include:

• Otitis media with effusion (OME) which occurs when mucus and fluid persist or continue to build up in the middle ear after an infection has resolved. It may also occur following some dysfunction or noninfectious blockage of the Eustachian tube.
• Acute otitis media, this types comes on quickly and is accompanied by swelling and redness in the ear.

Diabetes mellitus is a metabolic diseases characterized by hyperglycemia resulting from defects in insulin secretion.

This leads to hyperglycemia, which may result in acute metabolic complications such as
 Diabetic ketoacidosis (DKA) &
 Hyperglycemic hyperosmolar nonketotic syndrome (HHNS).

Diabetic ketoacidosis is characterized by hyperglycemia, ketonemia, and acidemia.DKA is caused by an absence or markedly inadequate amount of insulin.
Causes of Diabetic Ketoacidosis
• Infection: (pneumonia, bacteremia, UTI etc.).drug intoxication
• Side effects of drugs (Steroids),Failed to take insulin
• Vascular occlusion ,Surgical procedures ,Stress
• Pregnancy
Diabetic Ketoacidosis : Pathophysiology
Insulin controls the glucose level in the blood by regulating the production & storage of glucose. In the diabetic state, the cells may stop responding to insulin or the pancreas may stop producing insulin.
Without insulin, the amount of glucose entering the cells is reduced & the liver increases glucose production.—-Hyperglycemia—–Osmotic Diuresis—–Dehydration and marked electrolyte loss.
Another effect of insulin deficiency
Breakdown of fat into FFA & glycerol——–FFA converted into Ketone bodies by the liver——-Accumulation of ketone bodies in the circulation
Metabolic acidosis
Clinical Manifestation of Diabetic Ketoacidosis
• Dehydration: Poor skin turgor, Dry mucous membrane
• Electrolyte imbalance
• Acidotic breathing
• Drowsiness, Restlessness, Stupor, Convulsion
• Cardiac irregularities
• Urine output high or low
Diagnostic Evaluation
• History collection
• Physical Examination : Signs of dehydration
• High blood sugar more than 300-400 mg/dl
• Blood Investigation :
Serum electrolytes: Sodium and potassium levels may be low, normal, or high, depending on the amount of water loss (dehydration).
Elevated BUN and creatinine
• ABG : Metabolic acidosis,Respiratory alkalosis
• Urine infections, Pus cells in urine, Ketone bodies in urine

Diabetic Ketoacidosis : Medical Management
• Rehydration:
• Restoring Electrolytes
• Reversing Acidosis
• Blood sugar control & Restoring vital organ control
Diabetic Ketoacidosis : Nursing Management
• Monitor vital signs ,saturation and Pain score
• Monitor electrolyte status as well as blood glucose levels.
• Monitor urine output and mainatin I/O chart.
• Monitor for dysrhythmias indicating abnormal potassium levels.
• Check the conscious level of patient.
• Administer insulin as per the order.

Think of the liver as a factory that is functioning 24/7.
It’s a factory that has a production line, waste disposal system and a very well run storage system.

The manufacturing component of the liver produces
A) Bile juice, which is critical in the digestion and absorption of fat in the small intestines. Bile also helps in the disposal of waste via our stool
B) Albumin and blood plasma proteins are essential in preventing leakage of fluid out of our blood vessels and in ensuring that our blood can clot to prevent excessive bleeding.
C) immune components and cells such as complement components and acute phase proteins which helps us get rid of external invading agents.

It also has excellent storage capabilities.
It stores the excess glucose floating in our bloodstream and pack them into glycogen within the liver, allowing our human body to utilise this energy when we need it.
It stores essential minerals and vitamins like vitamins A, D, E, K and B12 as well as iron and copper.
All of these minerals and vitamins are vital to our bodily functions, including the formation of blood haemoglobin.

And perhaps the most important function of the liver is in its filtration and waste disposal capability.
All the blood leaving the stomach and intestines passes through the liver, which removes toxins, byproducts, bacteria and other harmful substances, which include drugs that we consume.
Ammonia, which is produced from all organs in our body, is removed almost entirely by the liver.
And If allowed to accumulate, high levels of ammonia can be toxic to our brain, causing confusion, hallucinations and even coma.
Our red blood cells renew themselves every 3 months. Old and dying red blood cells are broken down and the yellowish bilirubin is removed via our liver as well.

So, if you can imagine, what will happen to us if our liver fails?
We will no longer be able to remove wastes and toxins. When this waste builds up, it poisons our organs from within and our body will shut down and stop functioning.
Our blood vessels become leaky with fluid accumulating in our skin and subcutaneous tissues, resulting in a swollen face, swollen abdomen and swollen legs.
In severe cases, water accumulates in our lungs, causing the sensation of drowning.
Our immune system will become severely weakened and we will be prone to all sorts of infection and we may start to bleed easily as our clotting function becomes impaired.
And we will start turning yellow because of the accumulation of bilirubin in our system.
This is known as jaundice

Now let us discuss more jaundice.
Jaundice occurs when high levels of yellowish bilirubin accumulate in our bloodstream, turning the white of our eyes and our skin yellow.
There are many causes of jaundice, which is a lecture by itself.
Jaundice can occur due to the overproduction of bilirubin in our bloodstream – such as when there is overwhelming destruction of our red blood cells.
Or it can occur due to the blockage of the bile duct by stones or cancers, resulting in bilirubin failing to be discharged in our stool. The bilirubin will then overflow back into our bloodstream.
In the majority of cases, it is often due to the inflammation and infection of the liver, of which viral hepatitis is the top culprit.
Hepatitis A, B, C and E are endemic in Singapore. Meaning they are around us all year round.
Inflammation of the liver by viral hepatitis can severely impair the liver’s ability to remove bilirubin.
And therefore jaundice is one of the prominent signs of advanced viral hepatitis.

Upon viral infection, the immune system therefore induces insulin resistance in order to increase insulin production by the pancreas, which stimulates the antiviral immune response. In lean people this is not a problem, because the pancreas easily compensates for temporary insulin resistance. But in obese people, who usually already have insulin resistance, infection may overload the ability of the pancreas to compensate, resulting in diabetes mellitus type 2.

Animation Description: Under normal circumstances, bacterial infection results in the release of chemokines that attract circulating neutrophils to the endothelium. This process is known as chemotaxis.

A variety of molecules are expressed on the endothelial cell surface that allow the neutrophil to be captured, then roll along the endothelium, then adhere.

Following adherence, the neutrophil migrates into the subendothelial tissue to reach the site of infection.

The neutrophil engulfs the bacteria and eliminates them via breakdown within the phagosomes — a process known as phagocytosis.

In states of hyperglycemia, chemotaxis is reduced. Adherence is also adversely affected.
Phagocytosis is also impaired by hyperglycemia.

Hyperglycemia also adversely affects the macrophage system. Under normal circumstances, circulating monocytes are attracted to sites of infection, roll, adhere, and then migrate into the subendothelial space. The monocyte then transforms into a macrophage.

which is then activated by cytokines released by the bacteria. The activated macrophage then engulfs the bacteria.

However, hyperglycemia results in decreased activation of macrophages, thereby arresting the process of macrophage phagocytosis of bacteria.

In addition to affecting neutrophil and macrophage function, hyperglycemia also affects the complement cascade. Under situations of normal glycemia, bacteria can activate the complement cascade.

Activation of the complement cascade results in the formation of transmembrane protein channels known as membrane attack complex (MAC) in bacterial membrane.

Membrane attack complexes make the bacterial membrane porous and the rapid influx of fluid results in the bacterial cell death.

Hyperglycemia inhibits the proper activation of the complement cascade, thereby reducing another pathway of the immune system.
